MISSION AND VISION STATEMENT

UIHS is the primary health provider for American Indian Families in the Humboldt and Del Norte Counties of California. The service area covers approximately 5,000 sq. miles with the exception of the Hoopa Reservation which has its own health program.

The American Indian population of the service area counties include several groups of Traditional tribes: Yurok, Karuk, Wiyot, Wylacki, Tolowa, Siletz, Hupa, and others. UIHS also provides services to American Indians from all areas of the United States who are residing in our service area.

SITES

The main UIHS facility is a full service outpatient clinic, the Potawot Health Village, which is located in Arcata, California (Humboldt County) near the Mad River Community Hospital.

This program consists of Community Service, Child & Family Services (CFS) Department.

We have two satellite health stations and two senior nutrition sites in Del Norte County: the Howonquet Indian Health Station located on the Smith River Rancheria and the Klamath Indian Health Station located on the Resighini Rancheria.

In addition, there is an alcoholism counseling center located on the Elk Valley Rancheria, which also houses our CHR, and Health and Wellness, Promotion and Education component.

SERVICES

Medicine - Total family health care: complete physical examinations for adults, health and vision testing, glaucoma testing, baby checkups and shots (immunizations), TB skin testing, pap smears, breast examinations, birth control counseling, pregnancy tests, abortion counseling, prenatal care and obstetrics, podiatry, physical therapy and allergy counseling & testing. Professional inpatient care is provided by UIHS staff physicians who maintain staff privileges at all local hospitals for purposes of admitting and treating our patients.

Service providers: 4 physicians, nurse practitioners and physician's assistants, 1 consulting physical therapist, and 1 allergist.

The satellite clinics in Del Norte County are staffed by 1 physician, 1 physician's assistant, a family nurse practitioner, and medical assistants.

Vision - Complete eye examinations, treatment of eye problems, minor surgery of the eye, referral for eye glasses ( which can be obtained through the optician), and eye glass repair.

Service providers: 1 ophthalmologist, 1 optometrist, and 1 optician.

Pharmacy - A full-service pharmacy that stresses patient consultation and drug information for clients; located at the Potawot Health Village. Pre-packaged medications are provided by the pharmacy for providers who see patients in Smith River, Klamath, Fortuna and Weitchpec.

Service providers: 2 full-time pharmacists, 1 on-call pharmacist, three pharmacy technicians, and 1 pharmacy assistant.

Nutrition - Nutrition education and counseling programs: a WIC program at Tsurai Health Center that extends to 12 sites, a Smith River Howonquet Senior Nutrition Site, and a Resighini Senior Nutrition Site.

Service providers: 2 public health nutritionists, a registered dietician, support staff

Dental - Modern facilities and up-to-date procedures are available at both the main clinic in Arcata (Potawot Health Village) and the satellite clinic in Smith River (Howonquet). These services include all procedures necessary to restore and maintain optimal dental health such as fillings, extractions, dentures, crowns, bridges, root canals, and preventive programs.

Arcata - 3 dentists, 2 hygienists, 5 dental assistants

Smith River - 1 dentist, 2 dental assistants, 1 dental hygienist

Health and Wellness - The health promotion and education section focuses on tobacco education, HIV/AIDS education, substance abuse prevention education, perinatal education, community mobilization and youth peer-to-peer educational programs and activities.

Service providers: 4 health education specialist/technicians, 2 clerks, 1 health promotion and education coordinator

The preventative health section offers community screening services, information and education, outreach services and community awareness. The focus is on diabetes issues.

Service providers: A Public Health Nurse supported by physicians, FNP's, PA's, Nutritionists, and Counselors from other programs.

The fitness program not only provides aerobic activities but helps the communities establish fitness activities that make sense to the community.

Service providers: A fitness coordinator who is supported by consultant aerobic instructors and community volunteers.

Community Health Services - Offers social services, environmental health services, and contract health services; liaisons with outside agencies; CHRs from Smith River, Crescent City, Klamath, Weitchpec, Pecwan, Arcata north, and Eureka south provide community and individual education and information, health screening, assistance in identifying and accessing resources for health needs, scheduled home visits, assistance in making appointments, and advocacy services

Service providers: 1 Public Health Nurse and 7 Community Health Representatives (CHR's)

Child and Family Services, located in Arcata (Potawot Health Village). This department provides individual, family and group counseling which focuses on the child in a family setting. For more information, call the UIHS central office at (707) 825-5000.
Service providers: 1 consultant psychologist, as well as marriage and family counselors with other expertise.
